The PISD Athletic Department would like to thank you and your student-athlete for filling out and submitting the required forms in a timely manner.

To get started, please have your student’s school ID number ready. You will have to enter “First Name”, “Last Name”, “Student ID”, and “School Attending”. The database that is used has been uploaded from PISD registration records. Please enter your student’s name as it would appear in his/her registration record. Do not use a “nickname”.

The following forms must be submitted before students are considered cleared for any athletic activities including practices and work-outs.

Please fill out and submit the following forms electronically:
• UIL Required Forms
• Emergency Medical Release
• Ponder Athletics Agreement

The UIL Forms, Emergency Medical Release, and Ponder Athletics Agreement can be accessed from the “Electronic Participation Forms” tab at the top of this page. Hold the cursor over the tab and you will see a drop down box with links to each of the required forms.

To begin, click on the form name and then follow the directions on each form. The forms must be filled out in their entirety.

To sign the document, click inside the signature box and hold the mouse down. This will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to signature box. If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

Please click on “I agree” next to the signature line. Once the information on each page has been completed, you will enter your email address (for confirmation) and then press “Submit”. * If you do not press submit, the form will not be captured in the Rank One system. Once the form has been submitted, you will receive a confirmation email.

Ponder Junior High students will not be required to fill out the Steroid Form or the Ponder Athletics Agreement Form.

Please submit a paper copy of the following:
• UIL Physical & Medical History Form

The required annual Pre-participation Physical Evaluation-Medical History Form (physical & medical history) cannot be filled out electronically because it must be completed by the parent/guardian and a physician. You may print this form by selecting the “Download and Print” tab at the top of the page. Hold the cursor over the tab and you will see the link to the “Pre-Participation Physical Evaluation” which you can then download and print. Once the form has been completed and signed by a physician, please turn it in to the athletic trainer or to the athletic secretary.
